Abstract

1. Phanh môi trên bám bất thường có thể gây ra một số vấn đề về thẩm mỹ và có mối liên quan đến sức khỏe răng miệng như: bệnh nha chu, lệch lạc răng ... Chính vì vậy, phanh môi trên bám bất thường có chỉ định điều trị phẫu thuật.. - Tại Việt Nam, nghiên cứu phanh môi trên bám bất thường còn rất ít. Vì vậy, nghiên cứu của NCS ở nhóm học sinh 7-11 tuổi là cần thiết. Trong thời gian gần đây, Laser Diode đã được sử dụng trong đó có phẫu thuật ở lĩnh vực Răng Hàm Mặt ngày càng nhiều. Phẫu thuật có sử dụng Laser được cho là có nhiều ưu điểm, tuy nhiên cũng cần phải có nhiều nghiên cứu để chứng minh điều này. Do đó, nghiên cứu của NCS cũng có giá trị thực tiễn.. - Ưu điểm của luận án là đã đi sâu nghiên cứu hình thể giải phẫu phanh môi trên trên một quần thể nghiên cứu lớn, mặc dù là một chi tiết rất nhỏ trong khoang miệng nhưng đã được tác giả khắc hoạ rõ nét và nêu bật được ảnh hưởng của giải phẫu bất thường. Bên cạnh đó tác giả cũng đã góp phần minh chứng thêm cùng với các công trình khác về tác dụng ưu việt của Laser Diode, một phương tiện được nha sĩ lựa chọn ngày càng nhiều trong thời kỳ nha khoa hiện đại.. 2. Luận án đã có những đóng góp mới cho chuyên ngành Răng Hàm Mặt, góp phần rõ hơn bức tranh tổng thể vể tình trạng hình dạng, ví trí bám của phanh môi trên cũng như mối liên quan của phanh môi trên bám bất thường với răng và nha chu của hai răng cửa giữa hàm trên.Nghiên cứu được thiết kế hợp lý với hai phần để đạt được các mục tiêu:. + Nghiên cứu mô tả cắt ngang: mô tả thực trạng phanh môi trên bám bất thường và mối liên quan với răng và nha chu của hai răng cửa giữa hàm trên ở học sinh 7-11 tuổi tại hai trường tiểu học Hà Nội.. + Nghiên cứu can thiệp lâm sàng mở không đối chứng: đánh giá hiệu quả điều trị phanh môi trên bám bất thường vào lợi dính (độ II) và nhú lợi (độ III) theo phân loại của Mirko Placke năm 1974bằng Laser Diode ở nhóm đối tượng có chỉ định điều trị.. + Cỡ mẫu được tính toán cẩn thận theo công thức phù hợp với từng phần nghiên cứu.. + NCS đã xây dựng các bộ phiếu thu thập thông tin đầy đủ và chính xác: xác định vị trí bám của phanh môi trên, hình thể.... + Quy trình kỹ thuật phẫu thuật bằng Laser được mô tả đầy đủ, các tiêu chí đánh giá kết quả và theo dõi kết quả điều trị cũng chi tiết, đầy đủ và khách quan.. - Các số liệu trong nghiên cứu được xử lý khoa học bằng phương pháp thống kê y học cho độ tin cậy cao.Khía cạnh đạo đức trong nghiên cứu cũng được đảm bảo.. - Nghiên cứu còn cung cấp thêm những minh chứng về hiệu quả Laser Diode trong điều trị phẫu thuật phanh môi trên bám bất thường.. 3.Kết quả của luận án có thể sử dụng trong giảng dạy, nghiên cứu khoa học, đồng thời có thể ứng dụng ngay trong thực tiễn điều trị.Các kết quả của đề tài đã cung cấp những thông tin khoa học, hữu ích cho các bác sĩ lâm sàng có thêm được một sự lựa chọn mới trong điều trị phanh môi trên bám bất thường, nâng cao hiệu quả điều trị bệnh, góp phần vào chương trình chăm sóc và bảo vệ sức khỏe cho nhân dân..

1. Abnormal upper labial frenum attachment can cause a number of cosmetic problems and is related to oral health, such as periodontal disease, diastema between two upper central inscisors and so on…In some cases, abnormal upper labial frenum attachment should be needed surgical treatment. In Vietnam, there has not too many research on abnormal upper labial frenum attachment until now. It is necessary to do this research in group of students aged 7-11. In recent time, Laser Diode has been popularly used in medical field in general and in dentistry in particularly, specialty in the field of Maxillofacial Surgery. Laser-based surgery has been showed more and more advantages and more researches should be done to prove this. Therefore, this research also has practical value. The thesis has studied upper labial frenum attachment in a large research population, although it is a very small detail anatomy in the oral cavity, and its characteristics has been described clearly and be highlighted its effects of labial frenum attchment anatomical abnormalities to the two upper central inscisors’s gingivals and its eruptions by the author. More over, the author has also contributed to illustrate with other reseachers on the advantages of Diode Laser in treatment of abnormal upper labial frenum attachment, a method that more and more chosen by dentists in the modern dental period.. 2.The thesis has made new contributions to the field of Odonto-Stomaltology, contributing more clearly to the overall picture of the shape, position of the upper labial frenum attachment as well as the realtionship of abnormal upper labial frenum attachment on dental and periodontal of two upper central inscisors. The study is logically designed with two parts to achieve the goals:Descriptive cross-sectional study: describing the status of the upper labial frenum attachment and the relationship of abnormal upper labial frenum attachment to dental and periodontal of two upper central inscisors in 7-11-year-old students at two elementary schools in Hanoi.. + Intervention study: evaluate the effectiveness of treatment of the abnormal upper labial frenum attachmentlevel II and level III (Mirko Placke’s classification – 1974) by Laser Diode.. + The sample size is carefully calculated according to the formula appropriate to each study.. + Reseacher has built a collecting data sheets to collect information completely and accurately: determining the location of the upper labial frenum attachment, its shape…. + Laser-Diode surgical technique procedure for treating abnormal upper labial frenum attachment is fully described as well as criteria for evaluating treatment results are detailed, complete and objective.. - The data in the research is processed scientifically by the method of medical statistics for high reliability.. - Ethical aspect in research is also guaranteed.. - The research also provides more evidence on the effectiveness of Laser Diode in the treatment of the upper labial frenum attachment.. 3.The results of the thesis can be used in teaching, scientific research and can be applied immediately in treatment clinical practice.The results of the research have provided the useful and scientific information for dentists and clinicians a new option in the upper labial frenum attachment anatomy and its treatment of abnormal upper labial frenum attachment, improve the effectiveness of treatment and contribute to the community health care and protection program..
