Abstract

Những kết luận mới của luận án:. - Đề tài đã sử dụng hệ thống nội soi phóng đại (NSPĐ) (x 50 - 150 lần) giúp quan sát chi tiết hơn cấu trúc bề mặt niêm mạc, mạch máu của polyp ĐTT.. - Đề tài là công bố đầu tiên ở Việt Nam đã thực hiện kết hợp phương pháp NSPĐ với các phương pháp NS nhuộm màu ảo (FICE), NS nhuộm màu thật (Indigo carmin, Crystal violet) và đánh giá giá trị của các phương pháp này trong dự đoán mô bệnh học của polyp ĐTT.. Đối với NSPĐ nhuộm màu ảo FICE giúp đánh giá hệ thống mạch máu niêm mạc polyp theo phân loại Teixeira gồm 5 type.. Đối với NSPĐ nhuộm màu thật Indigo carmin 0,2% và Crystal violet 0,05% giúp đánh giá đặc điểm hình thái lỗ niêm mạc polyp (pit pattern) theo phân loại Kudo.. Đề tài là tài liệu tham khảo đóng góp thêm bằng chứng khoa học về giá trị của các phương pháp NSPĐ, nhuộm màu trong chẩn đoán sớm polyp đại trực tràng ung thư hóa và mức độ xâm lấn, là bằng chứng khoa học giúp bác sỹ nội soi lựa chọn phương pháp điều trị phù hợp (EMR, ESD hoặc phẫu thuật). Hơn nữa, việc sử dụng hệ thống NSPĐ nhuộm màu ảo và thật cũng giúp phát hiện được những trường hợp polyp kích thước nhỏ (< 10 mm) nhưng mô bệnh học là loạn sản độ cao và ung thư.

New conclusions of thesis: The thesis used magnifying endoscopy system (x150 times) to help observe in more detail the mucosal surface structure, mucosal capillary vascular system characteristics of colorectal polyps. This is the first dissertation in Vietnam to combine magnification endoscopy combined with virtual color endoscopy (FICE), true color endoscopy (Indigo carmin, Crystal violet) to consider the validity of these methods in predicting histopathology (with neoplastic and neoplastic) of colorectal polyps.. FICE magnifying endoscopy (virtual chromoendoscopy) allows evaluating the vascular system of colorectal polyps according to Teixeira's vascular classification includes 5 types.Magnifying chromoendoscopy with Indigo carmin 0,2% and Crystal violet 0,05% allows evaluating of pit pattern morphology of polyps (pit patterns) according to the Kudo classification.Our thesis has contributed more data for the literature on the value of magnifying chromoendoscopy methods in early diagnosis of cancerous colorectal polyps and the degree of invasiveness. This is important scientific evidence that helps endoscopists choose the right treatment (EMR, ESD or surgery). Furthermore, the use of a virtual and real magnifying chromoendoscopy system also helps detect more small polyps (<10 mm) but the histopathology is high dysplasia and cancer..
