Abstract

Những kết luận mới của luận án: 1. Rối loạn phát âm gây ảnh hưởng nặng nề tới tâm lý, học tập, hoà nhập của trẻ khe hở môi vòm miệng. Chính vì vậy, trẻ sau phẫu thuật môi vòm miệng cần được điều trị ngữ âm - Tại Việt Nam, nghiên cứu về điều trị ngữ âm nói chung và điều trị ngữ âm cho trẻ khe hở môi vòm miệng còn rất ít. Vì vậy, nghiên cứu của NCS ở lĩnh vực này là cần thiết. Trong thời gian gần đây, mô hình điều trị toàn diện phối hợp đa ngành cho trẻ khe hở môi vòm miệng, đặc biệt là phục hồi chức năng phát âm sau phẫu thuật rất được chú trọng. Điều trị ngữ âm mang lại kết quả tốt nhưng cần phải có nhiều nghiên cứu để chứng minh điều này. Do đó, nghiên cứu của NCS cũng có giá trị thực tiễn. Ưu điểm của luận án là đã đi sâu nghiên cứu đặc điểm phát âm của trẻ sau phẫu thuật bao gồm đầy đủ các âm vị phụ âm, nguyên âm và thanh điệu. Bên cạnh đó tác giả cũng đã góp phần minh chứng thêm cùng với các công trình khác về vai trò và tác dụng của điều trị ngữ âm cho trẻ sau mổ, một nhu cầu và xu hướng hiện đại trên thế giới và đang được ngành Răng hàm mặt rất quan tâm. 2. Luận án đã có những đóng góp mới cho chuyên ngành Răng Hàm Mặt, góp phần rõ hơn bức tranh tổng thể vể điều trị toàn diện liên ngành đối với trẻ khe hở môi vòm miệng. Cũng như sự hỗ trợ phục hồi chức năng vòm miệng sau mổ bởi các phương pháp điều trị ngữ âm. Nghiên cứu được thiết kế hợp lý với hai phần để đạt được các mục tiêu: + Nghiên cứu mô tả cắt ngang: mô tả đặc điểm phát âm của trẻ từ 3-5 tuổi sau phẫu thuật môi vòm miệng tại Bệnh viện Răng hàm mặt Trung Ương Hà Nội. + Nghiên cứu can thiệp lâm sàng mở không đối chứng: đánh giá hiệu quả điều. trị ngữ âm bằng phối hợp liệu pháp âm vị với hướng dẫn cấu âm truyền thống tại bệnh viện và gia đình.. + Cỡ mẫu được tính toán cẩn thận theo công thức phù hợp với từng phần nghiên cứu.. + NCS đã xây dựng các bộ phiếu thu thập thông tin đầy đủ và chính xác: xác định âm sai, qui trình sai, tính dễ hiểu…. + Quy trình điều trị ngữ âm được mô tả đầy đủ, các tiêu chí đánh giá kết quả và theo dõi kết quả điều trị cũng chi tiết, đầy đủ và khách quan.. - Các số liệu trong nghiên cứu được xử lý khoa học bằng phương pháp thống kê y học cho độ tin cậy cao. Khía cạnh đạo đức trong nghiên cứu cũng được đảm bảo.. - Nghiên cứu còn cung cấp thêm những minh chứng về hiệu quả điều trị ngữ âm trong phục hồi chức năng vòm miệng sau phẫu thuật.. Kết quả của luận án có thể sử dụng trong giảng dạy, nghiên cứu khoa học, đồng thời có thể ứng dụng ngay trong thực tiễn điều trị. Các kết quả của đề tài đã cung cấp những thông tin khoa học, hữu ích cho các bác sĩ lâm sàng có thêm được một sự lựa chọn mới trong điều trị toàn diện trẻ khe hở môi vòm miệng, nâng cao hiệu quả điều trị bệnh, góp phần vào chương trình chăm sóc và bảo vệ sức khỏe cho nhân dân.

New conclusions of the thesis: 1. Speech language disorders have serious impacts on the psychology, learning and integration of children with cleft lip and cleft palate. Therefore, the children after cleft lip and cleft palate surgery should be given the phonetic treatment. - In Vietnam, the study on phonetic therapy in general and phonetic therapy for children with cleft lip and cleft palate is very limited. Therefore, the research students in this field is very necessary. In recent times, the comprehensive multi-disciplinary combination treatment model for children with cleft lip and cleft palate, especially for rehabilitation of speech after surgery, has been focused. The phonetic therapy has shown good results, but further studies are needed to prove this. Therefore, the Author’s research is also of practical values. The advantage of the dissertation is that it has studied in depth the phonetic characteristics of children after surgery, including full consonant sounds, vowels and tones. In addition, the author has also contributed to further prove along with other works on the role and effect of speech language therapy for children after surgery, a modern need and trend in the world and being in the Odonto-Stomatology are ery concerned. 2. The thesis has made new contributions to the specialization of Odonto-Stomatology, contributing more clearly to the overall picture of the comprehensive interdisciplinary treatment of children with cleft lip and cleft palate as well as the support for postoperative oral rehabilitation by the phonetic therapies. The research has bene logically designed with the two parts in order to achieve the objectives: + The descriptive cross-sectional study: Describing the pronunciation characteristics of children aged 3-5 years after cleft lip and cleft palate surgery at National Hospital of Odonto-Stomatology Hanoi. + Uncontrolled open clinical intervention study: Evaluating the effectiveness of phonetic therapy by combining phonemic therapy with traditional phonetic instruction at hospitals and families.. + The sample size is carefully calculated according to the formula suitable for each research part.. + The research student has developed complete and accurate information collection forms: identifying false sounds, wrong process, ease of understanding .... + The phonetic treatment process is fully described, the criteria for evaluating the results and monitoring the results of treatment are detailed, complete and objective.. - The data in the study are scientifically processed by the medical statistical method with the high reliability. The ethical aspects of the research is also guaranteed.. - The research also provides additional evidence of the effectiveness of speech language therapy in restoring palate function after surgery.. The results of the thesis can be used in teaching, scientific research, and at the same time can be immediately applied in the treatment practice. The results of the thesis have provided scientific and useful information for clinicians to have a new choice in the comprehensive treatment of children with cleft lip and cleft palate, improving the disease treatment efficiency, contributing to the program of health care and protection for the people.
