Thực trạng thừa cân, béo phì và hiệu quả của một số giải pháp can thiệp ở học sinh tiểu học tại thành phố Bắc Ninh

Abstract

Những kết luận mới của luận án:. Tại Bắc Ninh, đây là nghiên cứu đầu tiên được triển khai để xác định được tỉ lệ thừa cân, béo phì (TCBP) và yếu tố nguy cơ, các bệnh lý kèm theo của TCBP ở học sinh tiểu học năm 2016. Bên cạnh đó, nghiên cứu thực hiện can thiệp đầu tiên tại Bắc Ninh về giải pháp kết hợp các biện pháp can thiệp tư vấn truyền thông thay đổi kiến thức, hành vi, lối sống và hướng dẫn thực hành chế độ ăn, hoạt động thể lực hằng ngày cho trẻ TCBP nhằm cải thiện tình trạng TCBP của trẻ. Ngoài ra, đây cũng là nghiên cứu đầu tiên tại Việt Nam thực hiện đánh giá chất lượng cuộc sống của trẻ TCBP thông qua hình ảnh AUQUEI.. Luận án có những kết luật đóng góp mới cho tình hình TCBP ở trẻ em lứa tuổi học sinh tiểu học, khi đã chỉ ra được thành phố Bắc Ninh khác các thành phố ở tốc tăng trưởng kinh tế xã hội dẫn đến tỉ lệ TCBP ở trẻ em gia tăng nhanh chóng (năm 2014 tỉ lệ TCBP ở học sinh tiểu học là 23,7%, sau 2 năm đã tăng lên 27,2%); Làm rõ hơn các yếu tố nguy cơ liên quan đến TCBP ở lứa tuổi này; Chỉ ra được các bệnh lý kèm theo như rối loạn mỡ máu, tăng đường máu, tăng huyết áp, gan nhiễm mỡ, bệnh răng miệng, bệnh hô hấp, cận thị; Đã đánh giá được chất lượng cuộc sống của trẻ TCBP; Kết hợp các biện pháp can thiệp hằng ngày cho nhà trường, gia đình và học sinh, đặc biệt hoạt động thể lực có đánh giá lượng hóa cụ thể, so sánh trước sau can thiệp, kết quả sau can thiệp đã cải thiện tình trạng rối loạn mỡ máu, đường máu, chỉ số BMI giảm, hiệu quả can thiệp thực sự đối với nhóm trẻ TCBP là 7,3% (riêng với nhóm béo phì là 19,2%), kết quả nghiên cứu của luận án rất có giá trị cho các can thiệp cộng đồng và điều trị bệnh nhi bị TCBP nặng sau này..

New conclusions of the thesis:. This is the first study has been conducted in Bac Ninh to determine the rate of overweight, obesity and risk factors, associated diseases of overweight and obesity among primary school pupils in 2016. This is also the first intervention study in Bac Ninh city on the combination of communication and counseling interventions to change knowledge, behavior, lifestyle and guide daily practice of diet, physical activity for children with disabilities in order to improve the status of the children with overweight and obesity. At the same time, this is also the first study conducted in Vietnam on the assessment of the life quality of children with overweight and obesity through AUQUEI image.. This thesis had new conclusions on the prevalence of overweight and obesity among primary school pupils, and pointed out the difference of Bac Ninh city, where is different from other cities in socio-economic growth, leaded to an rapid increase in the proportion of overweight and obesity in children (that proportion was 23.7% in 2014, after 2 years it increased to 27.2%); This also provide more clarification of risk factors related to overweight and obesity at this age; Point out the associated diseases such as dyslipidemia, hyperglycemia, hypertension, fatty liver, oral disease, respiratory disease, near-sightedness; Moreover, this thesis also showed that the life quality of these children reduced; Combination of daily interventions for schools, families and pupils, especially the physical activity with specific quantitative assessments, pre- and post-intervention comparisons, post-intervention results revealed improvement in the situation of dyslipidemia, hyperglycemia, BMI decreased, the effectiveness of interventions for the children with overweight and obesity was 7.3% (for obese group only was 19.2%), the research results of the thesis are very valuable for community interventions and treatment of pediatric patients with severe overweight and obesity thereafter..
