Những kết luận mới của luận án:. - Nồng độ procalcitonin ở bệnh nhân nhiễm khuẩn ổ bụng nặng/sốc nhiễm khuẩn cao nhất tại thời điểm sau phẫu thuật và giảm dần trong quá trình điều trị. Procalcitonin có mối tương quan thuận mức độ trung bình với CRP nhưng không tương quan với nhiệt độ và bạch cầu. Ở các bệnh nhân sống sót, procalcitonin giảm nhanh hơn so với CRP nên procalcitonin phù hợp hơn để tiên lượng và theo dõi đáp ứng điều trị sau can thiệp phẫu thuật.. - Nồng độ procalcitonin tại thời điểm ban đầu sau phẫu thuật có giá trị thấp trong tiên lượng biến chứng và tử vong, nhưng độ thanh thải procalcitonin sau 48 giờ (giảm <50%) và sau 7 ngày (giảm <80%) có giá trị tốt trong tiên lượng biến chứng và tử vong.. - Độ thanh thải procalcitonin sau 48 giờ và sau 7 ngày có giá trị tiên lượng tương đương với giá trị tiên lượng của độ thanh thải lactat sau 48 giờ và các thang điểm APACHE II và SOFA, nhưng cao hơn có ý nghĩa so với độ thanh thải lactat sau 7 ngày. Chỉ có độ thanh thải procalcitonin sau 48 giờ và sau 7 ngày là các yếu tố độc lập có giá trị tiên lượng tử vong và biến chứng sau phẫu thuật trong phân tích hồi quy đa biến.. Cán bộ hướng dẫn PGS.TS. Summary of new main scientific contribution of the thesis:. - The serum procalcitonin levels in patients with intra-abdominal sepsis/septic shock were highest on admission in ICU after surgery and decreased in the following days. Correlation coefficient between procalcitonin and CRP was determined as moderate. However, there was no significant correlation between procalcitonin and temperature as well as white blood cell. In patients without complications, the levels of procalcitonin declined more rapidly than the levels of CRP, so procalcitonin showed a more favorable kinetic profile than CRP to predict the clinical outcome.. - The change of procalcitonin concentration reflected as procalcitonin clearance aſter 48 hours (<50%) and 7 days (<80%) from baseline, but not the baseline concentration, were associated with prediction of complications and mortality in patients with abdominal sepsis/septic shock.. - The prognostic values of procalcitonin clearance after 48 hours and after 7 days were as good as the prognostic values of APACHE II and SOFA score and lactate clearance after 48 hours but were significantly higher compared with lactate clearance after 72 hours.. - The procalcitonin clearance after 48 hours <50%, and after 7 days <80%, rather than absolute initial procalcitonin concentration, were the independent prognostic factors of mortality and postoperative complications in the multivariate regression analysis Name of supervisors A.Pr.
